Community Homebound Outreach Collaborative

Texas Oral Health Coalition

The community homebound outreach collaborative (choc) provides an opportunity for TxOHC to partner with non-dental organizations like insurance companies and the local area agency on aging (AAA) and meals on wheels (MOW) programs to provide oral health materials and resources to help address social determinants of health factors and foster whole-person health for this targeted population.

In efforts to increase oral health knowledge for individuals that are homebound and their caregivers, members of the Coalition’s older adult committee developed If Your Mouth Could Talk rack cards in English and Spanish with helpful oral health tips for older adults related to aging, dry mouth, and dementia.

The TxOHC Community Homebound Outreach Collaborative (CHOC) was established after partnering with the Alamo Area Council of Governments (AACOG) Area Agencies on Aging (AAA), Meals on Wheels San Antonio (MOWSA), and UnitedHealthcare to provide oral health kits with educational materials and supplies to over 3,600 Meals on Wheels recipients in Bexar, Frio and Atascosa counties in October 2019.  

In collaboration with UnitedHealthcare and the Harris County Area Agency on Aging Houston Health Department, Human Services Division, CHOC partners disseminated oral health kits and resources to 8,200 homebound meal recipients in Houston Harris County in October 2020.
